Abuse is a violation of an individual's human and civil rights.
Abuse may consist of a single act or repeated acts and may affect one person or more. Abuse can take many forms the following are only examples. It may be physical, verbal, psychological, or discriminatory, it may be an act of neglect or an omission to act, or it may occur when a vulnerable person is persuaded to enter into a financial or sexual transaction to which he or she has not consented, or cannot consent. Abuse may take the form of honour based violence, modern slavery or exploitation.

Abuse

An indication of the type of accommodation that a client currently has. This should be based upon the client’s main or permanent residence.

Accommodation Status

The activity or set of activities taken by the CASSR as a result of the initial safeguarding concern, or subsequent enquiry.

Action

A range of hospital settings and can include specialist rehabilitation settings.

Acute / long-term healthcare residential facility or hospital (e.g. NHS Independent general hospital / clinic, long-stay hospital, specialist rehabilitation / recovery hospital)

Advocate

Age is calculated as at the last day of the reporting period. If a client dies during the period, their age should be recorded as their age at death.

Is the age requirement met, i.e. the person is or will be 18 years old or older at the start of the proposed DoLS authorisation.

When the long term support service provided to a client comes to an end and no other long term support is being provided. This could be the result of the clients needs being re-assessed and there is no longer the need to provide the long term support or the support is temporarily suspended for other reasons such as a hospital admission.

Premises intended for individuals released from prison or other types of custodial detention, who are usually ordered to stay there as part of their release from prison, or as a condition of bail or a Community Order.

This section includes the cost of telecare service contracts as well as equipment and adaptations. No further financial analysis is required between Primary Support Reasons or age groups.

Asylum seekers are defined by the United Nations as people who move across borders in search of protection, but who may not fulfil the strict criteria laid down by the UNCHR 1951 Refugee Convention.
Asylum seeker describes someone who has applied for protection as a refugee and is awaiting the determination of his or her status.

Is the best interests requirement met, i.e. it is in the person's best interests to be deprived of their liberty in this care home or hospital for the purpose of giving the proposed care or treatment, it is necessary to prevent harm to that person, and it is proportionate to the risk and seriousness of that harm.

The loan is intended to cover the period until a property is sold to pay for care rather than a loan for the lifetime of the holder.

Capital charges i.e. depreciation, loss of impairment of assets, credits for capital grants and Revenue Expenditure Funded from Capital under Statute (REFCUS) should be recorded under service related expenditure (columns D and E) and as capital charges (columns P and Q).
Capital charges and other residual costs associated with the closure of establishments should be included in FR007: Commissioning and Service Delivery Measure (columns D and E) and as capital charges (columns P and Q).
Capital charges i.e. depreciation, loss on impairment of assets and credit for amortisation of capital grants should be recorded in the ASC-FR. It should be noted that credit for amortisation of capital grants is included, but the element of notional interest should be excluded from capital charges.

The process of assessing and reviewing a client's needs and arranging service delivery.

Care management

The definition of a carer is taken from the Carers and Disabled Children Act 2000 which utilises the following description: “Carers (aged 16 and over) who provide or intend to provide a substantial amount of care on a regular basis for another individual aged 18 or over”. Although the Act only refers to carers aged 16 and over, younger carers of adults should be included in this return. The Act excludes from the definition of a carer, paid care workers and volunteers from a voluntary organisation.
It is possible for a client to have more than one carer, and for a carer to additionally be a client in his or her own right.

Carer

Indicator of client’s responsibilities for caring for another person, i.e. ‘carer’; or ‘not a carer’.

Carer role

The Carer Support Reason describes the needs of the carer. The carer’s support needs may change based on their personal circumstances and on the adult receiving care needs.

Changes in the capacity or ability of the carer to provide support to the client. This may be a temporary issue relating to the carer, but the local authority is required to review the client to cover the gap in support or a more long term issue such as the death of carer.

Council with Adult Social Services Responsibility.

CASSR

A client with a package of services provided by the CASSR but not within a personal budget, with no cash payments.

CASSR Commissioned Support only

A client in receipt of a personal budget where none of their personal budget is being taken as a direct payment.

Specialist NHS children and young people’s mental health services.

Child and Adolescent Mental Health Service (CAMHS)

Social services support, care and protection for vulnerable children and young people. This is the most common route of transition to Adult Social Care.

Customers of the CASSR who are ‘on the books’ for an assessment, a review or the receipt of services.

Client

Income received as a result of Deferred Payment Agreements should be captured in this section as with any income from Client Contributions.

If the DPA-holder did not meet the eligibility criteria but was offered an agreement anyway (including a top-up), they should be classed as discretionary.

Commissioning and Service Delivery

Community refers to  those who live independently (including those in small group homes, sheltered housing or warden supported accommodation) who receive home or community based services.

Community

This route of access reflects requests from clients (or on behalf of clients) based in the community, residential / nursing care or any other route of access not specified elsewhere.

Community/Other route

A place providing care and recreation facilities for those who cannot be fully independent.

Day centre or day activities

A chronic or persistent disorder of the mental processes caused by brain disease or injury and marked by memory disorders, personality changes, and impaired reasoning

Dementia

Direct payment describes a payment process where support is given via the issue of monetary payments by local CASSRs directly to adult clients who have been assessed as needing certain services. Examples here could be: the issue of a personal budget solely via direct payment of funds to the recipient, or the issue of a grant payment to support a carer.

Direct Payment only

This route of access relates to clients who are being referred for support following a planned or an emergency admission to hospital.

Discharge from Hospital

Includes abuse based on a person’s race, sex, disability, faith, sexual orientation, or age; other forms of harassment, slurs or similar treatment or hate crime/hate incident. 

Discriminatory abuse

This relates to requests for support relating to clients who are being referred as a means of preventing admission to hospital. Diversion will include some kinds of falls prevention and falls response services, as well as reablement type services aimed at avoiding hospital admission. It is intended to pick up those referrals made specifically to prevent admission to hospital. CASSRs will have well-defined programmes for preventing admissions, such as reablement.

Diversion from Hospital Services

An incident or pattern of incidents of controlling, coercive or threatening behaviour, violence or abuse by someone who is or has been an intimate partner or family member regardless of gender or sexuality. It can include: psychological, physical, sexual, financial, emotional abuse; ‘honour’ based violence; Female Genital Mutilation; forced marriage. 

Domestic abuse

A DPA is a means by which an individual can ‘defer’ or delay accessing the equity in their home to pay for their care. The local authority pays the costs of care on an interim basis and is repaid at a later date - either after the sale of property (or other security) or after the person receiving care has died.

DPA (Deferred Payment Agreement)

Following a means test the client is a self funder, and has agreed a DPA for the full costs of their care.

DPA for full cost of residential or nursing home place

Deferred payment agreement, where no third party contributes to the cost of care.

DPA with no top up

A deferred payment agreement where a third party contributes an amount on top of the DPA to cover the cost of care.

DPA with top up

The value associated with DPAs which have been partially recovered or written off during the year.
• The value written off for DPAs where recovery was attempted but no value recovered
• The value written off and value recovered for DPAs where recovery was attempted and a partial value was recovered
• The value written off for DPAs where the Local authority did not attempt recovery

A combination of hearing and visual impairments.

Dual impairment

This is when a client’s short term support ended early and long term services restarted, in any setting.

Early cessation of service (leading to long term support)

All other instances of short term support to maximise independence ending early where long term social care support does not resume. This is not expected to be a commonly used sequel as most ‘early cessation’ clients are expected to go on to receive ongoing health care or resume council support in their original setting.

Early cessation of service (not leading to long term support) – other reason

A period of short term support to maximise independence that was cut short in an unplanned way (e.g. because of a health issue, client moving out of the local area etc.) and does not result in long term support provision.

Early Cessation of Service (not returning to long term support) - NHS funded care/end of life/deceased

A period of short term support to maximise independence that was cut short in an unplanned way before the planned end date but this resulted in provision of long term services.

Early Cessation of Service (return to long term support) - any setting

Is the eligibility requirement met, i.e. the person is eligible for the DoLS to be used, rather than the Mental Health Act 1983, to authorise a deprivation of liberty.

All service users, at the point data are extracted, who are in receipt of long term support services funded or managed by the council following a full assessment of need. This is the same population of service users who would be reported in table LTS001b of the SALT return.

Eligible Population (ASCS)

End of life (EOL) care is considered to be any episode of social care support provided as part of palliative care, and which is intended to support the individual until the care is no longer required.

Section 17 of the HASSASSA Act 1983 provides that councils may recover
such charges as they consider reasonable in respect of relevant services.

Fairer Charging Income

Includes theft, fraud, exploitation, pressure in connection with wills, property or inheritance or financial transactions, or the misuse or misappropriation of property, possessions or benefits. 

Financial or material abuse

A full cost paying client is one who pays the full direct costs of the services they receive but whose support is arranged by the CASSR which includes regular reviews, support planning etc.

Grants to voluntary organisations to enable them to provide a service should be included under the column ‘Grants to Voluntary Organisations’ (Non SALT & Totals worksheet Column F). Additionally, record in this column any expenditure for which you are unable to match activity. Such grants should be recorded against the most appropriate finance measure, e.g. grants to the Third Sector aimed at supporting people in the community through low-level support or advocacy should be included in the Information and Early Intervention measure.

Gross Current Expenditure

Includes things like ICT, HR, property and finance costs, which may be directly or indirectly assigned to Primary Support Reasons. These should be included in the SSMSS column, so that the actual service cost for comparison purposes can be deduced. Some overhead costs are charged directly, e.g. ICT equipment, phone charges, but these are true costs of running the business and aren’t generally included in SSMSS (where they aren’t charged directly they may end up as part of the overall ICT overhead included in the SSMSS column, but these differences would not be material enough to affect unit costs).

The United Nations World Health Organisation (WHO) defines hearing impairment as the complete or partial loss of the ability to hear from one or both ears and can be graded as mild, moderate, severe or profound.

Home care is care that allows a person with needs to stay in their home. It might be for people who are getting older (aging in place). It could also be for people who are chronically ill, recovering from surgery, or disabled.

The individual at risk could be a patient or a visitor.

This significant event relates to reviews carried out after an admission to or treatment in hospital. Even when a hospital admission or treatment was known about in advance, the result of medical treatment may be unexpected and result in a request for a review to the CASSR. If an unexpected admission to hospital or treatment in hospital is required, this may result in the care needs of a client changing when they are due to return home

Hospital (Planned and Unplanned episodes)

Can include places like community centres, day care centres, leisure centres, libraries, schools, GP surgeries and dental surgeries. 

In a community service (location of risk)

Can include places like businesses, offices, pubs and other people’s homes. 

In the community (excluding community services) (location of risk)

All income received from NHS bodies which has been spent on the provision of Adult Social Care. This includes funds received as part of a joint arrangement with an NHS body. Income received by the Local Authority as part of the Better Care Fund should also be recorded under Income from the NHS.

The number of individuals involved in the different types of safeguarding activity. Individuals involved refers to the adults at risk.

Individuals Involved in Safeguarding Activity

When an adult at risk dies from or suffers from serious harm, a SAR is conducted to identify how local professionals and organisations can improve the way they work together. A Safeguarding Adults Board (SAB) would usually make the decision to instigate a SAR and a report will be produced to document the findings and recommendations. This is a count of individuals involved in SARs which concluded during the reporting year. Individuals involved refers to the adults at risk.

Individuals Involved in Safeguarding Adult Reviews

Any expenditure on any service or support for which there is no test of eligibility and no requirement for review. The expenditure for this measure relates to the following items:
Information and Advice: spend on advice and publication teams, leaflets and advertising, websites and other information channels
Screening and Signposting: investment in contact centres, one stop shops, advice services etc.
Prevention / low level support: non-attributable costs for drop in centres, supported luncheon clubs, falls prevention and low level brokerage services (e.g. supporting access to full costed services such as gardening or shopping)
Independent Advocacy: Costs of supporting advocacy and associated functions.

Information and Early Intervention

A person who interprets, especially one who translates speech orally or into sign language

The SEA states that in the case of pooled budgets and joint arrangements, only the CASSR’s own expenditure contribution should be recorded; this is also the case for Care Trusts

Utilised within the SALT collection this classification is used to report the circumstances of the assessment of the carers needs. Here the carer was assessed jointly with the cared-for person. The level of client involvement will vary, case by case and between councils. Nevertheless, if the cared-for person is known, linked to the carer on the client database and was involved in a significant way in the assessment / review of the carer, this should be included under this classification.

Other – Known to Individual and Other – Unknown to Individual cover all other sources of risk which are not social care support. The source of risk would be classed as known to individual if the adult at risk knows their name.

Known to individual

Following a means test the client is entitled to some LA funding to partially cover care costs, and therefore are not classed as self funders. The LA pays part of the clients care costs, and the individual has deferred the remaining care costs.

Professionals must work in line with the Mental Capacity Act (MCA) and where an MCA assessment concludes an individual lacks capacity to make decisions about the safeguarding incident, decisions made on their behalf must always be in their best interest.

Lacking Capacity

Primary Support Reason that should be utilised in relation to clients who need assistance to  understand new or complex information. Learning Disability Support covers a very wide spectrum. The following list gives examples of types of support that could be included in this classification:
• help to keep safe and free from harm or neglect 
• assistance to live independently 
• support with social and educational activities 
• help with communication
• support for work or training 
An individual may have a learning disability but would not be recorded under Primary Support Reason ‘Learning Disability support’ if their primary reason for support identified at assessment is identified as falling under one of the other classifications.

Learning Disability Support

Community based users whose long term support resumes at a lower level following completion of short term support to maximise independence. It will also apply to nursing care clients whose decreased needs mean they ‘step down’ to residential care

Level of Long-Term Support Decreased

Community based clients whose care package increases following completion of short term support to maximise independence and the resumption of long term support. It will also apply to residential care clients whose increased needs require nursing care.

A lifeline alarm is a pendant or bracelet that is worn in the home and garden, which allows the wearer to get immediate assistance at the push of a button should they need it.

The deferred payment agreement is intended to be in place for the lifetime of the holder.

Lifetime loan

The loan amount is paid to the applicant who then pays for the cost of care. This differs from the traditional DPA type, where the Local Authority would pay the care costs directly.

Loan Style

Any service or support which is provided with the intention of maintaining quality of life for an individual on an ongoing basis, which has been allocated on the basis of eligibility criteria/policies (i.e. an assessment of need has taken place) and is subject to regular review.

Long term support

A health problem that requires ongoing management over a period of years or decades. It cannot currently be cured but can be controlled with the use of medication and/or other therapies.

Long-standing illness

A place where members of the community, particularly older or vulnerable people, come together to share a meal

MSP is about having conversations with people about how to respond in safeguarding situations in a way that enhances involvement, choice and control as well as improving quality of life, wellbeing and safety. The Care Act advocates a person centred rather than process driven approach and many local authorities have already adopted this way of working.

Is the mental health requirement met, i.e. the person has a mental disorder as defined under the Mental Health Act 1983, but disregarding any exclusion for people with learning disabilities.

Primary Support Reason which covers a very wide range of support for mental health and mental illnesses. The following types of support provide illustrative examples of the type of care that may be provided;
• Support with living independently in their own home, or having support to improve their home
• Support to go out, (perhaps with a personal assistant)
• Support to keep them motivated
• Someone to confide in, so they could talk over problems
• Support to travel independently

Mobile accommodation (for example caravans) utilised by the Gypsy, Roma and Traveller communities, irrespective of whether the accommodation is on a permanent, semi-permanent or transient site.

Encompasses slavery, human trafficking, forced labour and domestic servitude. Traffickers and slave masters use whatever means they have at their disposal to coerce, deceive and force individuals into a life of abuse, servitude and inhumane treatment.
